[1.6.7] ID exterminate quest summons

I've had the exterminate quest become impossible several times on several runs. The quest is not complete yet there is no monster anywhere on the level. What I've noticed this last time is that there was a summoner. Is it possible that a summon disappearing messes with the quest?

[1.5.10] InfD mirror image doesn't spawn

In the infinite dungeon, when getting the mirror image quest, the mirror image sometimes doesn't spawn. Ie: you can run across the entire level making every tile lit without finding the start the of the quest.
This has now happened to me for the second time.

When I entered scintillating caves, I started on the way to the second level. When I entered the second level, I started on the way to the third. On the last level I did start on the way back. The way back was still present in the first and second level, just not where I started.

About the card thing: this could lead to people using cards and shuffling before combat untill they have the cards left that they want. Note that with the second and third Aegis skills maxed and a bit of healing modifier, you can get as much or even more shield out of direct healing infusions than from shielding runes. (though they useally don't last as long)

There should be an icon with a lock somewhere, if you click it you can drag and resize a lot of things. Make the hotkey bar big enough and a second row will show.
